Overview
This document outlines the SMS opt-in requirements for Wyoming Pain Relief website forms. If any form on your site collects a phone number (such as Contact Us forms or appointment schedulers), you must include explicit consent language to comply with 10DLC regulations.
These requirements help ensure your texting registration remains approved and that patients provide express consent before receiving SMS messages.
When This Is Required
You must follow these steps if:
• Your website forms collect phone numbers
• You send appointment reminders or office notifications via SMS
• You are registering or maintaining 10DLC texting compliance
If a form requires a phone number, opt-in language and a checkbox are mandatory.
What You’ll Need
• Access to your website editor
• Link to your Privacy Policy page
• Link to your SMS Terms & Conditions page
Step 1: Add an Unchecked Checkbox
Place a checkbox directly next to or immediately below the phone number field.
Label example:
“Opt-in to Text Messages”
Important requirements:
• The checkbox must NOT be pre-checked
• The patient must manually click the box
• This confirms express consent (not implied consent)
Step 2: Add Required Compliance Language
Directly next to or below the checkbox, paste the following text (with links):
Opt-in to Text Messages: By providing a telephone number and checking “Opt-in to Text Messages”, I consent to be contacted by Wyoming Pain Relief via SMS text messages regarding customer care, appointment reminders, account notifications, and office communications. Reply STOP to opt-out of further messaging. Reply HELP for more information. Message frequency may vary. Message and data rates may apply. I agree with the Privacy Policy (INSERT PRIVACY POLICY LINK) and Terms & Conditions (INSERT TERMS LINK) of Wyoming Pain Relief.
Step 3: Verify Phone Number Field Settings
Check whether your phone number field is marked as Required or Optional.
Required Field:
If the phone number is required to submit the form, you MUST include the checkbox and compliance language.
Optional Field:
If users can submit without entering a phone number, this specific opt-in language is not strictly required, though still recommended.
Common Issues
• Registration denied: Phone numbers collected without opt-in checkbox and language
• Checkbox pre-checked: This is not compliant — must be unchecked by default
